• Key Responsibilities:

    Case Management

    •  Provide trauma-informed, culturally competent case management to clients preand post-release.
    • Assess client needs, strengths, and barriers to reentry.
    •  Develop individualized service plans addressing housing, employment, education, medical/mental health, and community support.
    • Connect clients with appropriate post-release programs and services, and monitor their engagement and progress over time.

    Release Planning

    •  Draft detailed, individualized release plans to support IRAA and compassionate release petitions.
    • Collaborate with attorneys, clients, and family members to gather supporting documentation.
    • Develop plans that demonstrate structured community support and readiness for reentry.

    Collaboration

    • Work closely with legal staff to align social work strategies with legal case objectives.
    • Participate in interdisciplinary case meetings and advocacy team strategy sessions.
    • Maintain thorough, timely case notes and documentation in line with organizational policies.

    Requirements
    Qualifications:
    • Master’s in Social Work (MSW) required; LGSW or LICSW preferred.
    • At least 2 years of relevant experience in case management, reentry services.
    • Experience working with incarcerated or formerly incarcerated individuals preferred.
    • Strong written and oral communication skills, including the ability to draft clear, persuasive release plans.
    • Ability to work collaboratively in a team-oriented, client-centered environment.
    • Ability to build trust with individuals impacted by trauma and the criminal legal system.
    • Strong organizational and time management skills, with the ability to manage multiple deadlines. Ability to work collaboratively and independently in a mission-driven environment.
