Key Responsibilities:
Case Management
- Provide trauma-informed, culturally competent case management to clients preand post-release.
- Assess client needs, strengths, and barriers to reentry.
- Develop individualized service plans addressing housing, employment, education, medical/mental health, and community support.
- Connect clients with appropriate post-release programs and services, and monitor their engagement and progress over time.
Release Planning
- Draft detailed, individualized release plans to support IRAA and compassionate release petitions.
- Collaborate with attorneys, clients, and family members to gather supporting documentation.
- Develop plans that demonstrate structured community support and readiness for reentry.
Collaboration
- Work closely with legal staff to align social work strategies with legal case objectives.
- Participate in interdisciplinary case meetings and advocacy team strategy sessions.
- Maintain thorough, timely case notes and documentation in line with organizational policies.
